Beginning April 1, 2009 new applications for the Ensure Program will not be accepted unless you are a participant in the Congregate Nutrition or Home Delivered Meals Program. To be eligible for ensure we must have your doctor’s approval. Your doctor must indicate that you have one of the following medical conditions: suffer from severe weight loss, are unable to chew and/or swallow solid foods, or undergoing chemotherapy or radiation. Your need for Ensure will be reassessed every six months. A new Ensure application and doctor’s form will need to be completed.  Ensure pick-up is limited to Tuesdays and Thursdays. You must call at least a day in advance to order. You can pick up your Ensure any time between 8am and 5pm on these days.  However, we will not be able to give out Ensure to you if you come in without calling ahead. Please call by 3:00 pm (Lexington 242-2290 or Thomasville 474-2754) on Monday to order for Tuesday pick-up, and on Wednesday to order for Thursday pick-up.  We can only provide Ensure for Davidson County residents age 60 and over who meet our eligibility requirements.  We CANNOT provide Ensure for anyone living in a nursing home, rest home, or other facility.
